Summary Statement:

Assists with the daily needs of the residents under the supervision of the licensed nurse. Complies with assisted living policies and procedures in accordance with federal and state regulations.

Essential Job Functions

• Provides transportation services to residents in all levels of care.
• Provides escort services as assigned to medical appointments.
• Schedules resident medical appointments.
• Provides and ensures proper safety procedures are used for residents in vehicles (straps for wheelchairs, seatbelts).
• Drives wheelchair van for medical appointments in absence of driver.
• Provides care services to residents within scope of practice and following community policies and procedures.
• Observes, reports and documents resident conditions to supervisor according to community polices and procedures.
• Participates in the development, implementation and evaluation of resident plan of care.
• Maintains a safe and clean environment for residents.
• Attends and participates in ongoing in-service programs.
• Performs other duties as assigned.

Job Requirements

Critical Skills and Knowledge Necessary

• Basic knowledge of health care needs of geriatric Resident
• Able to communicate effectively in English both verbally and in writing
• Able to follow direction and ask questions appropriately
• Ability to cooperate and interact with personnel from other departments, as well as with Residents and their families/visitors

The critical skills and knowledge required to successfully perform the functions of the position are normally obtained thru the completion of a high school education, a current state certification for Certified Nursing Assistant, and a minimum of three months experience within the geriatrics health care field. Maintain a required renewal and training for current certification and may be expected to obtain CDL license.

• Lifting 50 pounds and carrying 10-20 pounds short distances frequently
• Lifting greater than 50 pounds may require assistance from co-worker or the use of a lifting device
• Standing and walking frequently with occasional sitting
• Able to perform simple grasping and pushing/pulling 50 pounds frequently
• Reaching below knees to above head frequently
• Bending, kneeling and squatting occasionally
• Twisting and turning upper torso occasionally
• Talking and Hearing in person and on telephone
