Acts is currently seeking qualified candidates for a M.D.S. Care Coordinator/ Nursing Position. This position will dedicate 24 hours to MDS and 16 hours to nursing related tasks.

In this role, the MDS coordinator will be responsible for the Resident Assessment Process from admission through completion of the Minimum Data Set (MDS) and the Care Area Assessments (CAAs). The MDS coordinator serves as a liaison between the MCG, the payer and the corporate office.

Must be a Registered Nurse in good standing and currently licensed by the State. Previous experience in long-term care facility or other healthcare related facility is required. Prior experience completing MDSs and Care Plans is required. Must be knowledgeable of general rehabilitative and restorative nursing, medical practices, procedures, laws, regulations and guidelines governing long-term care.

You will be responsible for the care of elders and guests who move in to the nursing home from the initial assessment of the level of care required, to the coordination of the care with the staff, and to the maintenance of the care records. You will ensure Medicare compliance and accurate completion of the M.D.S. and serve as liaison between the nursing home, the payer, and the corporate office.

Job Requirements

The ideal candidate will meet the following requirements:

  • State Registered Nurse (RN) license
  • Current or eligible for certification in Cardiac/Pulmonary Resuscitation (CPR) and Intravenous Therapy
  • Minimum of 2 years’ experience in M.D.S. and delivery of patient care, preferably in Geriatric and/or Medical/Surgical Nursing
