Acts is currently seeking qualified candidates for our Community Memory Services Coordinator.

In this role, you will provide leadership and coordination of integrated, comprehensive services and support for individuals with dementia and their caregivers across all levels of care within the community. You will also be responsible for communicating, implementing and coordinating programs and initiatives aligned with Acts dementia care vision. This individual will work collaboratively across disciplines to ensure best practice and innovative care and services for residents with dementia and their caregivers and will contribute specialized expertise in behavioral health within the interdisciplinary team. Specific focus areas will include, but are not limited to, specialized memory care, brain health, behavioral intervention, caregiver education and support services, staff education and consultation, outreach activities, and implementation of dementia related programs, services, outreach, policies and projects.

Job Requirements

The ideal candidate will meet the following requirements:

Education Level: At a minimum, an MA in a mental health related specialty, e.g. social services, psychology or behavioral analysis.         Evidence of specialized training in gerontology and/or dementia also required.

Years of Relevant Work Experience: Minimum of 5 years’ leadership experience in long term care memory care, dementia related services and/or social services setting. Experience in providing education and training to professionals and stakeholders preferred.
