Acts is currently seeking qualified candidates for our Clinical Outreach Coordinator position working in Skilled Nursing at Park Point Village in Rock Hill, SC.

In this role, you will establish and maintain a referral base for Assisted Living and Skilled Nursing Care (called Oak Bridge Terrace - OBT- and WillowBrook Court - WBC) by meeting with and marketing to all potential lead referral sources in the market service area under the direction of the Director of Clinical Admissions. This includes, but is not limited to, medical and social referral sources for OBT and WBC; churches, financial planners, elder law attorneys, and any other professional who may refer a potential resident or adult child influencer to a potential move-in to OBT and WBC. You will also generate and process private pay admissions to increase occupancy, generate new referral accounts, and build/ maintain a short term waiting list for resident admissions. Other responsibilities will include maintaining the census target and budget levels as outlined and monitor occupancy. You will also be involved in resident transfers, discharges as well as assist with potential resident assessments as necessary for resident admissions. You will also provide monthly reports and some administrative responsibilities related to marketing activities and admissions.

Job Requirements

Education & Experience: The critical skills and knowledge required to successfully perform the functions of the position are normally obtained thru:

      • Education Level: The completion of a bachelor’s degree or equivalent work experience.
      • Years of Relevant Work Experience: 2-3 years of direct work experience in senior services and health care marketing/admissions in assisted living and skilled nursing centers.
      • Experience with Medicare guidelines, insurance verification, and AllScript software preferred.
