Summary Statement:


Assists in the care of convalescent, chronically ill, or disabled persons in the home under the direct supervision of a licensed nurse, through the performance of personal services in order to enable the resident to remain and function effectively at home as long as possible.

Essential Job Functions

1) Assists resident in personal care and hygiene, bath, care of hair, mouth, skin, and nails, as directed.
2) Assists in transfers in and out of bed, ambulation, simple prescribed exercises and use of equipment, wheelchair, hoyer lift, and commodes.
3) Cues resident to take oral medicine as directed by nurse.
4) Provides clean and healthful environment for resident.
5) Promotes the resident to achieve independence within his/her medically determined limitations.
6) Promotes mental alertness through conversation and appropriate activities.
7) Observes and reports deviations from usual mental and physical status and changes in family situations or needs to the supervisor.
8) Assists residents with nutritional needs, to include preparation of meal, adhering to special diets, assist with feeding and washing utensils, used in the course of the visit.
9) Follows established guidelines and performs duties per direction of the home health RN.
10) Identifies safety hazards and emergency situations and initiates corrective action immediately.
11) Attends mandatory education programs and in-services.
12) Performs the following, after receiving proper instruction:
    a) Measures and records fluid intake and output
    b) Measures and prepares special diets
    c) Assists with prescribed ice cap or collar
    d) Assists with prescribed range-of-motion exercises
    e) Assists with the change of a colostomy bag
    f) Reinforces dressings
    g) Assists with use of devices for activities of daily living
    h) Takes and records vital signs
13) Transports residents to appointments, as necessary
14) May perform other duties as assigned.

Restrictions - the Wellness Nurse Aide does not:
• Change sterile dressing
• Irrigate body cavities
• Irrigate a colostomy or wound
• Perform a gastric lavage or gavage
• Catheterize a resident
• Administer medications
• Apply heat by any method
• Care for a tracheostomy tube
• Perform laundry duties
• Provide transportation services of any nature

Job Requirements

This is a Full Time Flex position with varied hours based on needs of residents.


Critical Skills and Knowledge Necessary

• Basic knowledge of health care needs of geriatric population
• Able to communicate effectively in English, both verbally and in writing
• Able to follow direction and ask questions appropriately
• Able to cooperate and interact well with residents and their family members

The critical skills and knowledge required to successfully perform the functions of the position are normally obtained thru the completion of approved home health aide training program or have the equivalent verifiable training and experience.

Physical Demands

• Lifting 50 pounds and carrying 10-20 pounds short distances occasionally. Weight greater than 50 pounds require assistance from co-worker or use of lifting device
• Standing and walking frequently with occasional sitting
• Able to perform simple grasping and pushing/pulling 50 pounds frequently
• Reaching below knees to above head frequently
• Bending, kneeling, and squatting occasionally
• Twisting and turning upper torso occasionally
• Talking and hearing, in person and on telephone
• Clear speaking voice
• Near acuity and color vision
