Summary Statement:

Supervises and coordinates daily operations and staff in the home health agency.

Essential Job Functions:

1. Manages, evaluates and supervises care delivered by licensed and non-professional staff in the home environment to ensure proper delivery of health care to clients in the home environment.
2. Manages and supervises home health personnel from the interview process through termination, if necessary.
3. Performs admissions, medication management, supervisory visits of staff on a routine basis to monitor compliance with the written plan of care.
4. Assesses client's needs and revises the plan of care as necessary and communicates any changes to the direct care giver.
5. Counsels the client and offers physical, emotional and psychological support as indicated.
6. Confers with client's attending physician and communicates plan of care to him/her.
7. Monitors client's progress or lack of and/or health status changes, communicates same to physician and resident nursing staff.
8. Reviews client's chart/folder for completeness with reference to: service agreement, consent for service, care plan, documentation of HHA/companion, etc.
9. Follows through on clients concerns.
10. Maintains good rapport with RRN staff regarding client's placement on Wellness Program and evaluates future needs.
11. Assists with emergency staff changes and coverage of cases, as needed.
12. Attends the facility meetings and in-services as required and participates in self-development programs to maintain clinical and managerial expertise and current standards of home health practice.
13. Monitors payroll
14. Ensures that all the professional standards of nursing practice are maintained by all nurses providing care.

Job Requirements

Critical Skills and Knowledge Necessary

● Current state license as Registered Nurse.
● Minimum of 2 years recent experience in Home Health.
● Previous experience with residents in the home setting.
● Knowledge of Federal, State regulations governing Home Health Care.
● Good physical assessment skills.
● Comprehensive knowledge of nursing principles.
● Current or eligible for certification in Cardiac/Pulmonary Resuscitation (CPR) and Intravenous Therapy.
● Excellent interpersonal skills.
● Good oral and written communication skills in English.

Physical Demands

● Lifting 50 pounds occasionally and carrying 10 pounds frequently for long distances. For weights greater than 50 pounds assistance from co-worker or use of lifting device is required.
● Sitting and walking short or long distances frequently and occasional standing.
● Driving as needed per client request, can be up to 2 hours at one time.
● Pushing/pulling up to 50 pounds occasionally.
● Reaching below knees to above shoulders occasionally.
● Bending and squatting occasionally.
● Simple grasping, fine manipulation and writing frequently.
● Talking and hearing in person and on telephone
● Clear speaking voice.
● Near acuity, far acuity and color vision.

Environmental Conditions

● Inside - 100%.
● Exposure to outside weather conditions when traveling between residences/facilities.
● Hazards:
- Blood borne pathogens.
- Infectious Diseases.

Specific Vocational Preparation

Specific vocational preparation includes an occupationally significant combination of: vocational education, apprentice training, on-the-job training or essential experience in other jobs:
