Summary Statement:

Administers and assists with the coordination of services and care of the client, ensuring quality and cost effectiveness.

Essential Job Functions:

1. Conducts assessments to identify problems and determine the need for services and the eligibility for those services.
2. Develops, implements and monitors care management plan with client and family.
3. Reviews financial, legal and medical issues and initiates referrals as needed.
4. Arranges and facilitates communication between client, family members, providers and staff.
5. Educates and supports client and family in moving toward self-care and maximum independence.
6. Researches and provides information to client and family to make informed health decisions.
7. Serves as advocate and liaison for client and family.
8. Provides assistance to client with organizing and paying bills.
9. Completes and files insurance claims
10. Performs other duties as assigned

Job Requirements

Critical Skills and Knowledge Necessary:

•  Knowledge of regulations related to the delivery of home health care services
•  Strong planning and organizational skills
•  Effective written and verbal communication
•  Positive relationship building skills
•  Valid Drivers License

The critical skills and knowledge required to successfully perform the functions of the position are normally obtained thru a license or certification in health or human services with minimum two years experience in care management.

Physical Demands:

•  Lifting and carrying 10-20 pounds occasionally
•  Sitting and walking short or long distances frequently and occasional standing
•  Driving frequently, up to 2 hours at one time
•  Pushing/pulling up to 50 pounds occasionally
•  Reaching below knees to above shoulders frequently
•  Bending and squatting occasionally
•  Simple grasping, fine manipulation and writing frequently
•  Talking and hearing in person and on telephone
•  Clear speaking voice
•  Near visual acuity, far acuity and color vision

Environmental Conditions:

•  Inside
•  Hazards - bloodborne pathogens and infectious diseases
•  Exposure to outside weather conditions when traveling

Hours of Operation:

•  Non-Exempt
•  additional hours as necessary
