Acts is currently seeking qualified candidates for our Home Services Supervisor position.

In this role, you will supervise and assist in the coordination of care. The Home Services Supervisor receives referrals and makes initial home visit within 48 hours of start of service to determine type, appropriateness, and adequacy of services. Performs supervisory visits of staff on a routine basis to ensure quality and compliance.

Essential Job Functions:

1. Manages, evaluates and supervises care delivery by licensed and non- professional staff in the home to ensure proper delivery of home care services to residents/clients in the home environment.
2. Receives referrals and makes initial home visit within 48 hours of start of service to determine type, appropriateness, and adequacy of services.
3. Manages, evaluates, and supervises care delivered by licensed and non-licensed staff.
4. Coordinates care with all parties necessary to support the objectives outlined in the Plan of Care and Service Provision Plan.
5. Participates in case conferences.
6. Complies with licensure and documentation requirements and ensures that all the professional standards of nursing practice are maintained by nurses.
7. Performs supervisory visits of staff and client/resident on a quarterly basis to monitor compliance with the written plan of care. Implements staff in-services as needed.
8. Supports BSS role with efforts to recruiting and participates in interviewing, orientation, and manages disciplinary processes/actions with staff.
9. Assesses client’s condition and revises the Service Provision Plan and Plan of Care, as necessary, communicating any changes to all appropriate parties.
10. Reviews client’s chart for completeness and accuracy.
11. Assists with emergency staff changes and coverage of cases.
12. Attend facility meetings and in-services as required to maintain compliance and participate in self development programs to maintain clinical and managerial expertise with current standards of home care practices.
13. Ensures all professional standards of nursing practices are maintained by all nurses providing care.
14. Follows through on client /staff concerns and incident reports, as directed.
15. Participates in, develops and/or presents community educational events for clients/residents, family members, and staff.
16. Support office staff with direction on all client/resident cases and scheduling issues.

Job Requirements

The ideal candidate will meet the following requirements:

  • Current State license as Registered Nurse (RN)
  • Current CPR certification 
  • Minimum of two years' recent experience with home health or Home Care
  • Covid Vaccinated Preferred
