Summary Statement:

Responsible and accountable for completing accurate schedules according to the needs of both the clients and employees. Assists with or provides personal care services to clients when needed.

Essential Job Functions:

1. Responds to questions regarding service from prospective clients, families and referral sources.
2. Follows computer program, procedures and guidelines.
3. Responsible for scheduling, evaluating, and when appropriate, participating in the disciplinary process for direct care employees.
4. Ensures that only properly qualified, screened, and approved employees are assigned to clients.
5. Assist in verifying scheduled visits.
6. Assists in obtaining necessary documentation from direct care staff.
7. Participates in weekly case conferences with the nursing director or to review services, concerns, etc. about the care provided each client. Maintains an open line of communications with clients and staff.
8. Reports verbally and in writing any complaints, incidents, or accidents immediately to the supervisor of scheduling coordination.
9. Reports any permanent schedule changes or change in client condition to nursing director.
10. Attends all job related inservices and town hall meetings.
11. On call schedule coordinator rotation 
12. Provides personal care services to clients when needed.
13. Other duties as assigned.

Job Requirements

Critical Skills and Knowledge Necessary:

  • Candidate must be a Certified Nursing Assistant or Personal Care Aide 
  • Good multi-tasking, computer and clerical skills
  • Effective organizational and interpersonal skills
  • Good communication and customer service skills

The critical skills and knowledge required to successfully perform the functions of the position are normally obtained thru the completion of a high school education and a minimum of six months experience in scheduling. 

Physical Demands:
•  Lifting 25 pounds maximum occasionally
•  Carrying up to 10 pounds frequently for long distances
•  Sitting and walking short to long distances frequently, with occasional standing
•  Driving occasionally up to 1 hour at one time
•  Pushing/pulling occasionally up to 25 pounds
•  Reaching below knees to above shoulders and bending occasionally
•  Frequent simple grasping, fine manipulations and writing
•  Talking and hearing in person and on telephone
•  Clear speaking voice
•  Near and far acuity

Environmental Conditions:

•  Inside
•  Exposure to outside weather conditions when traveling

Hours of Operation:

• Non-exempt
• 8 hours per day/5 days per week
• Overtime if needed
• On-call scheduling coordinator rotation
