Summary Statement:

Provide professional, compassionate, personal care and related services in all hospice participant residences, as assigned by the RN Care Coordinator or Hospice RN.

Essential Job Functions (including, but not limited to):

• Promptly report pain or any signs or symptoms of discomfort, whether physical or psychosocial
Promote maintenance of hospice participant, personal hygiene by assisting with bathing (bed, tub or shower), shampooing, shaving, care of skin, hair, nails, and mouth, assisting with dressing and changing of bed linens
• Help participants get in and out of bed and assist with ambulation. May include help turning and positioning, dangling, or help transferring to chair, commode or wheelchair
• Assist with elimination needs including use of bedpan, urinal, disposable briefs, emptying of catheter or ostomy drainage bags, or assist to bathroom
• Assist participants with the use of durable medical equipment
• Assist with range of motion or other prescribed exercises, when required
• Perform homemaking duties as assigned by the hospice RN
• Plan and prepare nutritious meals in accordance with dietary restrictions
• Assist with feeding as needed and assigned by the hospice RN
• Attend interdisciplinary, staff and CNA meetings as required
• Maintain documentation as required by hospice policy, Medicare and accreditation requirements


Job Requirements

Critical Skills and Knowledge Necessary:

Demonstrated communication skills which include the ability to consistently listen and speak with compassion. The critical skills and knowledge required to successfully perform the functions of the position are otherwise normally obtained thru the completion of a high school education with the ability to read and write English, and successful completion of a certified nursing assistant program. On the job hospice training is required to learn hospice philosophy and policies and procedures. CPR certification and Delaware CNA certification necessary.

Physical Demands

• Ability to lift or shift 50 pounds.
• Frequent sitting, walking, standing, stooping, crouching
• Reaching from below knee to above shoulder.
• Ability to use both hands, independently or together.
• Talking and hearing both in person and on the telephone. Clear speaking voice.
• Close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus

Environmental Conditions

May be exposed to blood, body tissue and fluids, as well as other hazardous waste materials.

Hours of Operation

• 30 hours per week
