As a member of the interdisciplinary team (IDT), coordinates the planning and delivery of quality, cost-effective hospice care. Evaluates hospice services and ensures implementation of hospice care plan. Serves as an advocate and educator for improved care of all people who are terminally ill.

Essential Job Functions:

• Oversees and supervises the implementation of an interdisciplinary, individualized plan of care for each participant and family/caregiver unit, including assessment and management of pain and undesirable symptoms which affect comfort
• Serves as a liaison between the team, the Medical Director and Administrator regarding clinical problems and progress in care of hospice participants
• Directly supervises hospice nursing personnel
• Consults and collaborates with involved physicians, other IDT members and others involved in the participant’s care, to meet participant and family/caregiver needs and help to resolve expressed concerns
• Assesses appropriateness for hospice admission and recertification, in conjunction with the attending physician and hospice medical director, based on documented evidence of end-stage illness, current assessment and other hospice criteria
• Participates in evaluation of services and performance improvement activities
• Participates in hospice-related research planning and protocols as well as community education and in-service opportunities.
• Schedules and supervises meetings of the interdisciplinary team.
 • Performs other duties as assigned

Job Requirements

Critical Skills and Knowledge Necessary

• The critical skills and knowledge required to successfully perform the functions of this position are normally obtained thru RN licensure in the state of Pennsylvania, pursuit or completion of a BSN degree, and 2-3 years RN experience in hospice or a transferable, clinical venue such as home care, geriatrics, or oncology. On the job training of 3-6 months is required to learn hospice computer software and hospice policy and procedures. Certification in hospice and palliative nursing within two years of employment is strongly encouraged.
