Acts Hospice is currently seeking qualified candidates for our Hospice Social Worker position.

The Hospice Social Worker develops, provides, documents and evaluates individualized social work assessment, counseling, and casework services to hospice participants and families as directed by the plan of care.  The Social Worker counsels hospice participants and families on issues related to self-determined life closure, safe and comfortable dying, and effective grieving, in collaboration with the Interdisciplinary Team (IDT).  Rotating on-call responsibilities are required.

Requirements

The ideal candidate will meet the following requirements:

  • Completion of a graduate degree in social work with licensure in the state of Pennsylvania
  • Demonstrated competence in assessment, counseling, advocacy, resource management, and developmental and cultural diversity related to the end of life is necessary
  • Minimum of two years experience in hospice or a transferable, clinical venue of social work practice such as oncology, geriatrics, or community health, is required
  • On the job hospice training is required to learn hospice computer programs and agency policy and procedures

 

Essential Job Functions: 

• Serves as a member of the IDT, consults and collaborates with other IDT members and others involved in the participant’s care, to meet participant and family/caregiver needs.
• Develops and executes the psychosocial component of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished
• Communicates about hospice services to prospective participants, families and referral sources as needed
• Provides psychosocial assessment, counseling services and direct casework to hospice participants and their families/caregivers
• Supervises master’s level, social work field placements, as available
• Documents problems, assessments, goals, interventions, and participant and family outcomes achieved
• Serves as an advocate and educator on bereavement and other hospice related issues.
• Participates in hospice research planning and protocols
• Provides support to staff and volunteers who are coping with loss-related stress
• Orients staff and volunteers to the role of psychosocial services

Job Requirements

Critical Skills and Knowledge Necessary

• The critical skills and knowledge required to successfully perform the functions of the position are normally obtained through the completion of a graduate degree in social work with licensure in the state of Pennsylvania. Physical Demands

• Lifting up to 20 pounds occasionally.
• Sitting and walking moderate distances frequently.
• Able to hear and see and speak clearly.
• Ability to drive 1-2 hours per day.

Environmental Conditions

• Ability to tolerate varied environments.

Hours of Operation

• 40 hours per week, additional hours may be necessary
• Standard 8 hour shift is 8:00am - 4:30pm
• Shared on call responsibilities
