Azalea Trace is currently seeking qualified candidates for our Occupational Therapist position. Acts Signature Rehab is committed to clinical excellence within the therapy program.

In this role, you will be responsible for OT services to home health, inpatient and outpatient including evaluations, planning individual treatment programs and goals, actual patient treatments, assistant supervision, and discharge planning. OT to perform other related duties such as continual growth of department with screening, caseload management, community education, clinical organization, daily scheduling, and developing home exercise prescriptions in partnership with Fitness. Job Requirements

The critical skills and knowledge required to successfully perform the functions of the position are normally obtained through:

Minimum of a Bachelor’s degree in Occupational Therapy (OT)
Valid OT license or eligibility for license in the state(s) where services are rendered
3-5 years' experience in an outpatient setting to include independent, assisted and skilled living, and home health; Skilled nursing rehab environment preferred.
Experience in some of the following clinical areas is preferred:
- Cardiovascular
- Pulmonary
- Geriatrics
- Neurology
- Orthopedics
- Vestibular Rehabilitation
- Low Vision
- Pain Management (lower back pain - shoulder pain)
- Memory Care
- Allen Cognitive Leveling
- Aquatics
- Lymphedema
- Pelvic Health
Any specialty area certifications or experience is key to Azalea Trace!
