Acts is currently seeking qualified candidates for a Social Services Coordinator position. In this role, you will provide residents and their families with medically related social service intervention. You will assess the psycho social needs of residents; provide supportive services to residents and families, as needed; and will also participate in the admission process with the assistance and consultation of the Nursing Home Administer (NHA) and Director of Nursing (DON).


Job Requirements

Critical Skills and Knowledge Necessary

• General knowledge of social services required in medical/health settings.
• Good interpersonal skills.

The ideal candidate will meet the following requirements:


  • Completion of a Bachelor degree in social services or related field
  • 2+ years of long term care experience
  • General knowledge of social services required in medical/health settings
  •  General knowledge of Microsoft Office and Windows preferred
  • Good interpersonal skills required
