Summary Statement: TEMP POSITION (JULY 1, 2019 - NOVEMBER 30, 2019)
 
To develop and monitor a support plan for every assisted living resident.

Essential Job Functions

• Develops an electronic support plan in compliance with the PA personal care home regulations for each assisted living resident upon admission, annually and with any status change of the resident’s condition, which includes the medical, dental, vision, hearing, mental health or other behavioral care services that will be made available to the resident, or referrals for the resident to outside services if the resident’s physician, physician’s assistant or certified registered nurse practitioner, determine the necessity of these services.
• Conducts routine support plan conferences with the resident, family, and the various staff members of the interdisciplinary team and routinely updates the support plan as needed.
• Assists new residents during the transition period into assisted living. Identifies and documents on the support plan the resident’s individual care needs through completion of admission paperwork and visits to check on the resident’s adjustment, social needs, activity needs, problems or concerns.
• Communicates with and provides updates to the assisted living administrator, nursing staff, residents and families on changes in physical, emotional and / or cognitive status of assisted living residents.
• Trains and monitors assisted living staff to maintain the resident’s electronic support plan by adding documentation as an acute episode arises and removing any resolved issues from the support plan as necessary.
• Maintains quality assurance program for services provided.
• May perform other duties as assigned.

Job Requirements

Critical Skills and Knowledge Necessary

The critical skills and knowledge required to successfully perform the functions of the position are normally obtained thru a high school education; obtaining and maintaining a current PA state license as a licensed practical nurse or working a minimum of 2 years in a related long term care setting. Minimal on the job training is required to learn the electronic support plan system.
