Acts is currently seeking qualified candidates for the role of Support Plan Coordinator in our Oakbridge Terrace (Assisted Living) neighborhood.

In this role, you will be responsible to develop an electronic support plan in compliance with the PA personal care home regulations for each assisted living resident upon admission, annually and with any status change of the resident’s condition, which includes the medical, dental, vision, hearing, mental health or other behavioral care services that will be made available to the resident, or referrals for the resident to outside services if the resident’s physician, physician’s assistant or certified registered nurse practitioner, determine the necessity of these services. Conducts routine support plan conferences with the resident, family, and the various staff members of the interdisciplinary team and routinely updates the support plan as needed.

Job Requirements

The ideal candidate will meet the following requirements:

  • High school diploma or equivalent
  • Obtaining and maintaining a current State license as a Licensed Practical Nurse (LPN) or licensed social worker
  • Minimum of two years in a related long term care setting
