Summary Statement:

Reviews, compiles and maintains medical records of the residents in the medical care facility to maintain compliance with federal, state and local regulations and company policies and procedures.

Essential Job Functions:

• Prepares charts and maintains records of newly admitted and current residents, including completion of face sheet.
• Reviews medical records for completeness, assembles records into standard order and files.
• Tracks resident's medical records charts to alert doctors when a resident is due for an appointment. Assists in updating medical records charts.
• Locates, signs out and delivers medical records requested.
• Follows instructions of Director of Nursing/Nursing Home Administrator and/or Medical Records Consultant in respect to maintaining compliance of medical records.
• Performs general clerical functions as directed by the DON and/or NHA.
• Answers telephone and intercom calls and provides information of relays messages to residents and staff.
• Directs visitors to residents’ rooms.
• Schedules patients for routine tests with providers.
• Tallies resident's charge sheets and forwards them to the accounting coordinator. Makes up new charge sheets at the beginning of a new billing period.
• Places calls to laboratory, doctors, ancillary services, etc. as directed by Director of Nursing, Nursing Home Administrator or Charge Nurse.
• Xeroxes necessary paperwork.
• Thins charts periodically.
• Performs other duties as assigned.

Non-Essential Functions:

• Distributes mail, newspapers and flowers to residents.
• Transports residents in wheelchairs to locations within the facility.
• Requisitions supplies designed by Nursing Staff.
• Copies information such as resident's vital signs onto medical records.
• Schedules transportation for residents for outside appointments as directed by the nurse.


Job Requirements

Critical Skills and Knowledge Necessary

• High School Diploma/G.E.D.
• Basic typing skills.
• Excellent communication skills.

The critical skills and knowledge required to successfully perform the functions of the position are normally obtained thru the completion of a high school education and a minimum of six months essential experience in other jobs.

Physical Demands

• Lifting 20 pounds occasionally and 10 pounds frequently.
• Carrying up to 20 pounds occasionally for moderate distances.
• Sitting frequently with occasional standing and walking.
• Reaching below knees to above shoulders frequently.
• Bending, squatting occasionally.
• Able to use either hand to perform frequent grasping, writing and typing.
• Talking and hearing in person and on telephone.
• Clear speaking voice.
• Near acuity.
